The very idea of having to ask for birthday sex is insane to me. If I were involved with you for 3 months, I would be planning that whole evening and if you asked me about it, I would have to give you my evil eye while enlightening you to the fact you were blowing my birthday surprise.
Sounds to me like she was looking for an excuse to unload and run, so you gave her the opportunity. Bet there were lots of "signs" if you look back on it and the no sex thing (much less asking for birthday sex) was the least of them.

Don, you read the tarot cards wrong. You were obviously a friend-without-benefits (FWOB for short). You wanted more, she didn't and you didn't take the clear hints that she dropped that while she liked hanging out with you, she wasn't interested in shagging you.
Her reaction sounds a bit over-the-top, but I sense we might get a different version of the events from her, possibly one in which she likes to hang out with you, but became frustrated with your desires for more.
Here's the lesson to take away from this: if you want a sexual relationship and it isn't moving in that direction with every passing date, give up on that aspect. The woman isn't interested in shagging you, no matter how much she might enjoy your company.
Pre-emptive clarification: "moving closer" means a progression of increasingly sexual actions on both parts, not shagging on the first or second date. Kissing on the first date and kissing passionately on the second counts. A peck on the cheek and a week later a peck on the cheek closer to your lips doesn't count.
